Download Instructions:
When you click on the download link you must click on 'Save' and save this document to your computer. If you 'Open' the document in the browser window any information you add will not be saved when you email it to us. Downloading it will enable you to update it over time and retain a copy for your records.

If the option to save the document does not appear when you click on the link, there is another way you can download the file;

PC Users: Right-click the link (click on it with your right mouse button) and select 'Save Target As', in the dialogue window that opens select where you would like to save the file e.g. Desktop or My Documents.

Apple Macintish Users: Press the 'control' key and then click the link, from the menu that appears select 'Download Linked File', this will save the file in your downloads folder.
